NEW!!! Insurance Certificate Request Form

There has been a change in the process in requesting insurance certificates. A new form has been designed for clubs to complete and send to their District Association for approval. The District Association will then send that request form to the Insurance Broker, who will then complete the certificate and send back to the club and copy the district.
